Now, field-based case managers utilize PDAs (Personal Data Assistant) to link clinical systems health care providers and insurers. Case managers located in hospitals, subacute care and skilled nursing facilities (SNFs) across the New York metropolitan area complete essential clinical evaluations that are sent wirelessly to health care providers involved with the care of each patient. This enables managed care companies and post-acute care providers to have immediate access to the clinical information necessary to make critical health care decisions.
This groundbreaking system incorporates the latest technology and represents a breakthrough in the way health care providers and payers communicate during the discharge planning process, leading to more efficient post-hospitalization placements and improved clinical patient care. Upon completing the clinical forms, they are transmitted instantly to the main server which distributes the information to the appropriate entities via fax or e-mail.
