CareNext Goes Digital!

Now, field-based case managers utilize PDAs (Personal Data Assistant) to link clinical systems health care providers and insurers. Case managers located in hospitals, subacute care and skilled nursing facilities (SNFs) across the New York metropolitan area complete essential clinical evaluations that are sent wirelessly to health care providers involved with the care of each patient. This enables managed care companies and post-acute care providers to have immediate access to the clinical information necessary to make critical health care decisions.

This groundbreaking system incorporates the latest technology and represents a breakthrough in the way health care providers and payers communicate during the discharge planning process, leading to more efficient post-hospitalization placements and improved clinical patient care. In addition to the capability of completing clinical evaluation the PDA platform enables both the nursing home and case manager to communicate instantaneously. Upon completing the clinical forms, they are transmitted instantly to the main server which distributes the information to the appropriate entities via fax or e-mail. The CareNext Connect system is compliant with all federally mandated HIPAA regulations.

With CareNext Connect the most current patient information is available in real-time to the clinical team at the post-acute care facility. Having accurate, timely clinical information enables the facility to provide efficient and effective clinical care, and ensures that needed equipment and medicines will be at the bedside upon admission.
