CareNext, L.L.C. is the premier provider of postacute care in the Northeast. Our comprehensive network consists facilities located in New York City as well as Nassau, Suffolk, Westchester and Rockland counties in New York State.

Postacute care is designed to treat patients who are recovering from an acute illness, injury or debilitating medical condition, and need extensive rehabilitation services and/or medical treatment. It is more intensive than traditional nursing home care but less intensive than hospital care. It is a goal-oriented treatment implemented immediately following or instead of acute hospitalization. postacute care is a specifically defined program whose primary goal is to have the patient return to previous functionality as quickly as possible.

All CareNext Network member facilities have distinct postacute units and a designated staff who provide care based on guidelines set by the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO). The focus is on personalized attention and one-to-one medical treatment.

Network facilities are staffed by doctors, physical therapists, nurses and a wide array of specialists who utilize state-of-the-art diagnostic and treatment technologies. Our network has the capability to provide both the technology and human care that is needed to ensure a quick and full recovery. Instead of being relegated to a hospital bed, patients can receive the complex medical care they need at a subacute center.

Our staff and facilities provide a clear pathway to recovery. In our extensive Network of facilities, over 85% of the patients were discharged directly home.
